AP Testing Off-Campus Consent Form
The 2024 AP Testing Period will take place beginning Monday, May 6, 2024 and concluding on Friday, May 17, 2024. Testing will take place in three locations - De La Salle, Carondelet, and Shadelands Art Center. Students taking their exam(s) at Shadeland Art Center will need permission from a Parent or Guardian to attend the off-campus location.

Listed below are the dates and times the AP exams are being held at the Shadelands Art Center located at 111 N Wiget Lane, Walnut Creek, CA 94598. All other AP exams will be held on the CHS and DLS campuses.

In consideration for securing the arrangements, I hereby release and save harmless the school and its agents from any and all liability for any injury that may occur during the testing.
