• Parent/Guardian Volunteer Clearance

  • Before Volunteering

    All parents who serve as volunteers with access to students are required to comply with Diocesan Requirements. While you are volunteering at Carondelet, you are responsible for your own safety. Workers’ compensation benefits are not provided to volunteers. All documents (except LiveScan results, which will be sent to us directly) should be uploaded to this form. 

    Parent/Guardian Driver and Volunteer Clearance

    1. LiveScan Fingerprints
    All adults who work with and volunteer at the school must complete fingerprint scanning. The Safe Environment Program requires that any volunteer having contact with Carondelet students have a fingerprint clearance – this serves as a protection for both you and students. Carondelet uses LiveScan.

    Fill out the LiveScan request and take it to a location that offers LiveScan services. The location nearest to campus is The Postal Annex at Oak Grove and Treat. You will be responsible for the cost of the fingerprinting. Clearance will be submitted electronically to Carondelet. Once completed, this requirement will be good for all the years that your student is attending Carondelet.

    If you have already completed fingerprinting through the Oakland Diocese, you still must complete this step for Carondelet.

    2. TB Risk Assessment or Test
    The California Health and Safety Code (§121545) requires all school volunteers to be examined and found free of communicable tuberculosis once every 4 years. Carondelet is required to keep an up-to-date certificate on file for each volunteer.

    Please have the Adult Tuberculosis (TB) Risk Assessment Questionnaire certified by your healthcare provider or provide us with a copy of a current negative TB test. TB test results are good for four years from the test date.

    3. Mandatory Training
    Volunteers are required to take an online child abuse awareness training course, “Protecting God’s Children” in compliance with the Safe Environment Program at the Diocese of Oakland. If you have taken this training within the last three years, you can use that certificate for volunteering at Carondelet. If not, you can access the online training:

    • Go to virtusonline.org
    • Click “First-Time Registrant”
    • Click “Begin the registration process”
    • Select “Oakland Diocese”

    Once you have created a user ID you will be asked to select a school or parish. Please select “Carondelet High School” from the dropdown menu. When asked to the session you wish to attend, scroll down and select “Protecting God’s Children for Adults online training session.” You can self-register and take the training, which lasts about 90 minutes. This training is required every three years.

    Keep a digital or printed version of the certification of completion to upload to the Volunteer Coordinator.

    4. License and Insurance
    All adults driving students on behalf of Carondelet must submit a copy of their valid Driver’s License and Automobile Insurance Declaration page showing a minimum of $100,000/$300,000 liability coverage. Valid until insurance or drivers license expires. These copies will need to be resubmitted upon expiration.

  • Transportation for Team Parents/Guardians

    As a parent volunteer you may volunteer to drive on athletic events, field trips, or other school functions. You must provide a copy of a valid California driver's license and proof of insurance to the Director of Athletics and Human Resources via this form.

    If you do drive on behalf of Carondelet High School, we want to communicate the following important information:

    Safety

    Your safety and the safety of our students is paramount. Therefore, please exercise common safety rules; seatbelts are required, no cellphone use while driving, do not leave backpacks, electronic devices or any other valuables visible when unattended and lock your vehicle when leaving.

    Accidents

    If you have an accident immediately report the accident to your insurance carrier and to Carondelet High School. Obtain the name(s) and contact information of any other person(s) involved in the accident including their driver’s license number. Take pictures of everything, IE. Cars involved, stop signs, lights, intersection, etc. Write down the specifics of the accident (where, what, who, and when) and the phone and badge number of any attending police officers.

    Insurance

    By law, the insurance on your vehicle is always your primary coverage. CHS is insured for any liability issues related to this type of accident. However, our insurance policy is in place to protect the school. The school does not provide property damage insurance and is not responsible for damage sustained to your vehicle(s). Any expenses related to the repair of your vehicle or any other cost related to the accident WILL NOT be reimbursed by Carondelet High School. Our insurance carrier requires that you insure your vehicle for a minimum of $300,000 property damage and bodily injury limits per accident. To protect yourself and family, we suggest that you consider extending your coverage to reach these limits.

    Declaration and Agreement

    I affirm that I will carefully transport students under my care, including obey all traffic laws. When I volunteer to drive students, I hereby waive, release, discharge, and agree to hold harmless Carondelet High School, it’s officers, directors, agents, insureds, employees, or anyone acting for or on its behalf, from any liability for personal injury, death, or property damage of any kind or nature whatsoever arising out of or in the course of my participating in any Carondelet High School Athletic game, practice, field trip or off-campus event.
